Apply for membership<\/h3>\n\n\n\n

Membership application form<\/a><\/p>\n\n\n\n

  1. All ophthalmologists registered with the MP State Medical Council and practicing ophthalmology in the revenue division of Jabalpur will be inducted as regular life members (RLM)<\/strong>.<\/li>\n\n\n\n
  2. Associate life members<\/strong> \u2013 Ophthalmologists practicing ophthalmology elsewhere in India or abroad, having residence in Jabalpur division.<\/li>\n\n\n\n
  3. Post-graduate members<\/strong> \u2013 require a letter from the head of the department of Ophthalmology. Do not need proposal and seconding by regular life members.<\/li>\n\n\n\n
  4. Proposed and seconded by an existing life member is compulsory<\/em> for Life Member application (regular and associate).<\/li>\n\n\n\n
  5. Please attach a copy of your post-graduation degree and your MP Medical Council registration certificate (for RLM, ALM).<\/li>\n\n\n\n
  6. Send the completed form to our secretariat as under.<\/li>\n<\/ol>\n\n\n\n
